All transcript requests must be on an appropriate, signed form.
CURRENT STUDENTS - Your transcript requests are now being accepted via a Google form! Here is the form:
TRANSCRIPT REQUEST CURRENT STUDENT
ALL GRADUATES - Alumni must complete the form below.
1. Print it out.
2. Provide all of the required information.
3. Sign the form.
4. Return it to Guidance:
* drop it off at the main entrance.
* email the form as PDF to [email protected] or [email protected].
